NV-HAP Surveillance using Structured Electronic Clinical Data

Non-ventilator hospital-acquired pneumonia (NV-HAP) is the most common and deadly healthcare-associated infection, affecting approximately 1% of patients with a crude mortality rate of 15-30%. Despite a pressing need for surveillance, detailed yet subjective diagnostic criteria for NV-HAP make monitoring onerous and difficult to replicate. In this collaborative project with the University of Utah, a pilot EHR-based surveillance definition was applied to a large set of hospitals in the Veteran’s Affairs and HCA Healthcare networks with the aim of characterizing the global incidence and variability of NV-HAP. This study demonstrated the feasibility of applying an electronic surveillance definition for NV-HAP to EHR data and affirmed the high crude mortality rate associated with NV-HAP. We found that both the traditional surveillance definition and the electronic surveillance definition correlate moderately with physician diagnosis and with one another, belying the subjectivity and lack of specificity of pneumonia criteria.
