This study, Electronic Medical Record Support of Public Health: Diabetes Mellitus (ESP:DM), adds chronic disease surveillance capabilities to an EMR based system that identifies and reports individual cases of notifiable disease. In ESP:DM, we will design, test and deploy a generalizable, open-source, system to extract, analyze, and communicate summary health and medical management information from electronic health records to inform diabetes surveillance. These new capabilities will be built upon our current Center of Excellence product, Electronic Medical Record Support for Public Health (ESP).

Initial project planning is being done in close conjunction with the Massachusetts Department of Public Health and the CDC Diabetes Task Force
